Request for a Letter of Recommendation
In order to write a good recommendation, we need your help.
- Complete the following information and give it to us with an addressed, stamped envelop for each school and any particulars about what you’re applying for that we need to know.
- Give us at least two week’s notice.
- Remember, you should go to the teacher you performed with best with. If you were an an average or rather lazy student in our class, the recommendation should say as much.
Make sure you give me the name you are applying under.
When must this letter be completed?
Where are you applying (which schools), or what are you applying for (which scholarship)?
What do you feel most called to do in life? Why?
What do your friends and family generally say that your strengths are?
What is your intended college major?
What is your GPA? What grade do you have in my class today? What did you have any previous semester?
What essay or assignment are you most proud of from my class? Explain your enjoyment or success.
List at least three classes that you enjoyed or did well in and briefly explain why.
List any awards you have received, AP exams you’ve taken, or other exams (SATS, etc) that you’ve tested well in.
List
all athletics, extra curricular, or school club involvement (they do
not have to all be school related) and explain your role in the club team, or organization.
Briefly describe any community service, job experience, or internships that you’ve had.
List any special skills, languages spoken, things that make you special, etc.
List an school disciplinary action you’ve been under in your high school years.
Is there anything else that you would like the people reading this letter to know about you?
